How do I get my mortgage company to help us?

My husband and I own a 1600 squarefoot home that we purchased at the peak of the market for 215,000. We got a 30 year fixed and a 2nd mortgage. We both finished college and began our careers before purchasing the home. In the last year we have had a baby and have both had to take pay cuts. We were lucky this year because my mom was able to watch my son for free. Next year she wont be able to. Right now we are able to pay out bills just by the skin of our teeth, but with childcare being over $500 a month next year we wont be able to make our mortgage payment.
Everytime I call Countrywide (lender) I get placed on hold. What do I have to do to get them to help me?
